A major growth market for Orion Health is the Health Information Exchange (HIE); an electronic network that integrates healthcare organizations within a region, community or hospital. The passage of the American Recovery and Reinvestment Act (ARRA), and the implementation of the Health Information Technology for Economic and Clinical Health (HITECH) Act provisions within ARRA has accelerated the development of HIE’s in the United States. Identifying this opportunity, Orion Health has focused on taking a leading role in HIE’s, including innovations such as SaaS-based delivery.
Orion Health realized that its HIE clients needed to be able to deploy a solution quickly, with lower risk and without the up-front cost of hardware and software licenses associated with an on-premise system. To support this, they set about designing a SaaS platform which would host its HIE product for a growing number of clients.
The SaaS platform is a critical piece of infrastructure for Orion Health, with SaaS clients representing an increasing proportion of their overall business. Over 70% of sales in North America are based on the SaaS platform, and platform usage is growing at a rate of 200% per annum.
To be commercially viable, the HIE SaaS platform had to be cost effective to deploy, operate and grow as new clients were added. The clinical data managed by the system, and the resulting requirements for security and privacy, mean that the platform had to comply with a range of health and IT standards. It also needed to provide a high performance, resilient and reliable service that supported around-the-clock access to the critical patient data it contains.
The SaaS environment needs to provide three fundamental features to ensure business continuity for clients; High Availability (HA), Scalability and Disaster Recovery (DR). These require redundancy both within the primary data center, as well as at a remote data center which can be activated in the event of significant failure at the primary site.
While the Orion Health software can operate on a range of database management systems, Oracle was selected for the SaaS platform based on its scalability and its favored status within the US market. Running on HP hardware, the Orion Health HIE uses Oracle RAC under a Standard Edition license.
In order to provide disaster recovery, a copy of the production database needs to be maintained at the secondary (DR) site, typically by replicating data updates to the DR site as they occur in production.
Orion Health initially considered Oracle’s Data Guard product for this role, however the requirement for Enterprise Edition, with its significant additional license costs, quickly eliminated it as an option. They also considered SAN replication, however this required an expensive dedicated link between the primary and DR sites.
Orion Health also considered Dbvisit Standby as a smart alternative to Data Guard. They found that by using Dbvisit Standby in conjunction with Oracle Standard Edition they could attain the same levels of performance and reliability as Data Guard, but at a fraction of the cost. Dbvisit Standby uses a combination of efficient data packaging and compression to minimize the data passed between the sites, removing the need for expensive high-bandwidth links between sites.
Orion Health has bundled Dbvisit Standby into their SaaS solution and configured it to mirror its production database at the DR site. In the event that the primary site becomes unavailable, the failover process will switch to the DR site.

Dbvisit Standby assists Orion Health in attaining these standards. For example, ISO27001 requires separation of duties such that database administrators need to be able to move and manipulate databases, but not access patients’ clinical data. Dbvisit Standby supports this through its secure configuration controls.
Although the DR mode has never been initiated in production, a simulated failover to the DR site is a requirement for each go-live, as well as annually as part of the platform’s ongoing ISO 27001 compliance. This gives Orion Health significant confidence that the solution will perform flawlessly in the event that a failover is required.

Successfully building SaaS platforms for clients, as Orion Health has done, requires close attention to the quantum and timing of costs. Service subscribers pay on an operating model, so it is critical that Orion Health’s costs are aligned with those revenues wherever possible, and that large capital costs are avoided.

This combination significantly reduces the up-front and ongoing cost to Orion Health in licensing the software for their SaaS platform. This allows them to offer a more price competitive solution to their clients, and grow the platform cost effectively as demand increases.
